Cognitive function is one of the important aspects in the period of a child’s development. Cognitive dysfunction can be a risk factor of neuropsychological development disorders. The Mini-Mental State Pediatric Examination (MMSPE) is one of the instruments for early detection of cognitive dysfunction in children. The adaptation of MMSPE into Bahasa Indonesia version is currently not available. There are no cognitive function screening instruments that assess higher functions or executive functions as tested in MMSPE.  This study aimed to adapt the MMSPE into Indonesia version and prove the validity and reliability of the instrument. This was a cross-sectional study that recruited 90 children aged 6–14 years who attended elementary and junior high schools. The research was conducted at Public Elementary School and Public Junior High School in Jakarta, Indonesia, from May to October 2024. The sampling method was carried out using stratified random sampling. The translation process of the instrument consists of forward and back translation. It was carried out by two pairs of certified translators. Content validity was assessed by five related experts and presented in the form of content validity index for items (I-CVI) and content validity index for scale (S-CVI) values. Reliability a was assessed by the Cronbach’s alpha value.  The Indonesian version of the MMSPE instrument has an average score of I-CVI, S-CVI/Ave (average), and S-CVI/UA (universal agreement) of 1. All items of the Indonesian version of the MMSPE instrument had calculated r > r table with a Cronbach’s alpha values of 0.754 and 0.760 respectively at the first and second examination. The Indonesian version of MMSPE is a valid and reliable instrument for screening of cognitive function in Indonesian children aged 6–14 years old. Early detection of cognitive screening using the Indonesian version of the MMSPE helps identify children at high risk of cognitive dysfunction.
